Services

Our Caritas Diabetes Care Program's outpatient services provide comprehensive education and support for participants and their families. From diet and exercise information to medication and glucose monitoring, Saint Anne's specially-trained health professionals partner with participants to help them learn to self-manage their diabetes and decrease the risk of future complications.

Nationally certified by the American Diabetes Association, the Caritas Diabetes Care Program's diabetes nurse educator and registered dietitian/certified diabetes educator provide participants and their families:

  • Individualized assessment by a diabetes educator
     

  • Instruction in self blood glucose monitoring
     

  • Nutrition assessment and recommendations
     

  • Small group classes that cover such topics as: healthy eating, the importance of diet, activity and medication in glucose control, the emotional component of living with diabetes, travel planning, dining out, food preparation
     

  • Follow-up telephone call and office visits
     

  • Individualized nutrition and nurse consultations
     

  • Monthly diabetes education and support groups

Referral: A physician referral is required for diabetes outpatient services.

Interpreter Services: As with all hospital services, interpreters (any language) for non-English-speaking patients are available.

Insurance

Most health insurance plans cover all or at least a portion of this service. For those without insurance or whose coverage is insufficient, special payment arrangements can be made.
