Community Outreach Program |

The Community Outreach Program includes presentations on various health topics at councils on aging and community groups, vital sign screenings at health fairs, and bereavement and support groups related to the loss of a loved one.

1) Health screenings on blood pressure, glucose, and cholesterol. 2) Lectures on various health topics, such as hypertension, stroke, osteoporosis, Lyme disease and others.3) Development of health program based on community or individual needs utilizing skilled nursing, rehabilitative therapies, or telemedicine technology |
